FREE Mentoring and supportive services provided to youth ages 16 to 25 utilizing the Transition to Independence (TIP) Model. Services include case management and goal setting related to – Education, Parenting Skills, Social Supports, Physical Health, Community Engagement, Employment, Financial Planning, Emotional Wellbeing and Housing.

School Based Mental Health* – School based mental health services provided in various school districts within San Joaquin County focusing on reducing symptoms related to home and community based trauma exposure. Intensive mental health supports and supportive student groups are offered at different campuses to enhance student’s academic performance and reduce behavioral interruptions at school. In home supportive services offered to families in need of case management and parent coaching support.

The Suicide Prevention Program works with youth in Lodi, Tracy, Lincoln, Manteca, Escalon, and Linden Unified School Districts as well as Able Charter schools to bring awareness of Suicide Prevention and offer early intervention services to those at risk. Evidence Based Practices for suicide prevention include the Yellow Ribbon campaign, SafeTALK, Break Free from Depression, and CAST – Coping And Support Training. Services will include training for staff and students to assist youth in crisis. Mental Health Specialists are on various high school campuses within these districts and are trained to offer: depression screenings, support groups and referrals for medication and psychological consultations.
