Mentoring and Peer Support

FREE Mentoring and supportive services provided to youth ages 16 to 25 utilizing the Transition to Independence (TIP) Model. Services include case management and goal setting related to – Education, Parenting Skills, Social Supports, Physical Health, Community Engagement, Employment, Financial Planning, Emotional Wellbeing and Housing.

School Based Programs

S.A.F.E. – Identifying barriers and factors putting Lodi Unified students at-risk of poor academic performance and/or suspensions and expulsions. The SAFE model provides clinical services on school campus and home-based case management services to address the core issues of students and families that support students in improving their academic performance and reducing behavioral interruptions at school.

Suicide Prevention

The Suicide Prevention Program works with youth in Lodi, Stockton, and Tracy Unified School districts to bring awareness of Suicide Prevention and offer early intervention services to those at risk. Evidence Based Practices for suicide prevention include the Yellow Ribbon campaign, SafeTALK, Break Free from Depression, and CAST – Coping And Support Training. Services will include training for staff and students to assist youth in crisis. Mental Health Specialists are on various high school campuses within these districts and are trained to offer: depression screenings, support groups and referrals for medication and psychological consultations.

Youth Behavioral Plans
